The sick Game Master part of me, of course, is looking at it and trying to figure just how much damage it would translate to...and what would happen to the target if it wasn't removed, or was, messily...ick.
Hmm...2d10 initially; at least another 1d12 if it's just yanked out (or maybe the other way around; looks like it'd do more damage coming out than going in)...if it's left in, the wielder has a chance to control the target with not only the shaft, but also the rope; the target takes an additional 1d4 damage each round from that until the harpoon/spear's removed (and that's if it isn't magicked to do worse, or be a Harpoon of Wounding or something similar)...
